Fecal Microbiota Transplantation in Recurrent Clostridium Difficile Infections: Our Experience in Patients with and without Inflammatory Bowel Disease

چکیده
The gastrointestinal tract harbors approximately 100 trillion bacterial cells that exist symbiotically to maintain the integrity of the intestinal mucosa and the gut immune system [1,2]. Intestinal homeostasis is achieved by the symbiotic relationship between the host immune system and these microbes. Disruptions of this homeostasis predispose the gastrointestinal tract to increased incidence of enteric infections. This phenomenon is believed to play a role in several intestinal and extra-intestinal diseases such as asthma, atopic diseases, obesity, colon cancer, irritable bowel syndrome, and inflammatory bowel disease [2,3]. Antibiotic use and immunocompromised states have been associated with an increased incidence of Clostridium difficile infection (CDI) [4]. Recent studies have established the role of environmental factors (such as diet, antibiotic use, and NSAID use), and their effect on the gut microbiome in the development of inflammatory bowel disease (IBD) in genetically susceptible individuals [1,5]. Ulcerative colitis (UC) is characterized by continuous inflammatory changes of the superficial layers of the colon, while Crohn’s disease (CD) is characterized by transmural inflammation that can affect any area of the gastrointestinal tract in a patchy manner [6]. Similar gut dysbiosis that occurs in antibiotic use, has also been found in patients with IBD [4]. It is well established that patients with IBD have a higher incidence of CDI than the average population [1,5,7].

Fecal microbiota transplantation in recurrent Clostridium difficile infection in a patient with concomitant inflammatory bowel disease.
The use of fecal microbiota transplantation in recurrent Clostridium difficile infection and coexistent inflammatory bowel disease remains unclear. A 61-year-old man with ulcerative pancolitis was diagnosed with a third recurrence of Clostridium difficile infection, previously treated with metronidazole, vancomycin and fidaxomicin. Fecal microbiota transplantation of an unrelated healthy donor ...

Fecal Microbiota Transplantation (FMT) methodology has been progressively refined over the past several years. The procedure has an extensive track record of success curing Clostridium difficile infection (CDI) with remarkably few adverse effects. It achieves similar levels of success whether the CDI occurs in the young or elderly, previously normal or profoundly ill patients, or those with CDI...
متن کاملRefractory Clostridium difficile Infection Cured With Fecal Microbiota Transplantation in Vancomycin-Resistant Enterococcus Colonized Patient
The rates and severity of Clostridium difficile infections, including pseudomembranous colitis, have increased markedly. However, there are few effective treatments for refractory or recurrent C. difficile infections and the outcomes are poor. Fecal microbiota transplantation is becoming increasingly accepted as an effective and safe intervention in patients with recurrent disease, likely due t...
متن کاملDesigning fecal microbiota transplant trials that account for differences in donor stool efficacy.
Fecal microbiota transplantation is a highly effective intervention for patients suffering from recurrent Clostridium difficile, a common hospital-acquired infection. Fecal microbiota transplantation's success as a therapy for C. difficile has inspired interest in performing clinical trials that experiment with fecal microbiota transplantation as a therapy for other conditions like inflammatory...
متن کاملFecal transplantation indications in ulcerative colitis. Preliminary study
BACKGROUND AND AIMS Fecal microbiota transplantation is used with success in persistent (more than two episodes) Clostridium Difficile Infection; it has also gained importance and started to be used in inflammatory bowel disease. There are theoretical arguments that justify its use in ulcerative colitis or Crohn's disease. Based on our clinical cases we tried to evaluate the indications of feca...
